Please make payment of $350 for each booth area (plus $200 if an additional space is needed) payable to ACUMS and address to:

Conference Registration: $350
Additional Space: $200
Unable to Attend but Will Send Literature/Sample Display: $150

Addtional Sponsorship Opportunities: (check if applicable)

Tuesday Night Banquet/Cocktail Hour $400 (includes dinner for the vendor plus 1 guest—2 total)
Lunch/Coffee Break Sponsorship: $75.00 (includes recognition at your booth and at break)
Additional Lunch $15 (2 are included in the $400 fee inclusive of additional space fee)

The Awards Banquet will be a buffet.
